             Genealogy reports are a list of all down line members from
             which you have received or will receive commission on their
             purchases. It includes name, address, expiration date, and
             cumulative purchases amounts from that position.

             Although the report will include names found on all 9
             levels, it will indicate the actual matrix position number
             for the first 3 levels only. Tracing the relationship of
             members beyond 3 levels would require combining the reports
             of those members.

             Please send me a complete printed genealogy report of my
             commission matrix. I understand that the report will be
             printed on standard 8.5 x 11 fan-fold paper in condensed
             print with a maximum of 66 lines per printed page.

             I further understand that the charge for this report is
             $3.00 for the 1st page and $1.00 for each additional page 
             printed in the report, and that the minimum charge for the 
             report is $3.00 (must be included with order) even if said 
             report includes no names at all.

             I am including $3.00 with my order to cover the minimum fee
             for this report and I hereby authorize MSM to deduct any
             amount not covered by this payment from future commission
             check(s).
